Planning for adults with care and support needs

We, the council and the NHS want to plan care and support services for the next five years.

If you are aged between 18 and 64 you can get the plan off to a good start by completing this survey.

If you have a mental illness, a learning or physical disability, autism, a sensory impairment, a long-term health condition or brain injury or are a carer, we’d love to hear your views.

We are asking about what’s most important about the help and support you get and what is important in the future. Your views will help us shape our plan.
