Oxfordshire County Council would like to hear your views on smoking and local stop smoking services in Oxfordshire. Whether you are a smoker, have never smoked or an ex-smoker we are interested in your opinions.

This survey will take around 10 minutes to complete. Thank you for taking the time to complete the questionnaire - your responses are important to us and will help to us plan how we reduce the number of people who start and continue to smoke in Oxfordshire.

The county council is responsible for helping to improve the health of the population and smoking is the leading cause of preventable death and disease in the UK.
